Lessons Learned and Potential Long-Term Trends from the Pandemic

Moderator: Mike McCaslin
CPA, Somerset CPAs and Advisors
Mike provides services for health care clients in multiple states throughout the country. As a principal in our health care group, he has many areas of expertise which include the development of health care delivery systems and multi-physician specialty organizations, and the negotiation of physician and hospital relationships. Mike also specializes in joint venture and merger analysis and transactions, strategic planning, buy-in and buy-out structuring and restructuring, development of governance structures and succession planning structures. In addition, Mike assists clients with income distribution methodology design and change management, new services development, ambulatory surgery center development and other physician and health system consulting needs. Mike speaks on health care, accounting and practice management issues to physicians, hospitals and other medical organizations.
Mike believes that the key to success is to surround yourself with smart, capable people and then empower them to make full use of their talent and expertise.

Rebecca J. Farmer, MBA
CEO, Center for Sports Medicine & Orthopaedics
A native of Tennessee, who earned her MBA from Auburn University, Becky Farmer has made a significant impact as an innovator and dedicated leader as CEO at Center for Sports Medicine & Orthopaedics.
Under her direction since 2000, the organization has grown from 3 clinic and physical therapy locations to 10 clinic locations, 5 physical therapy locations, and an ambulatory surgery center. One of her many roles includes physician recruitment and she has strategically expanded the staff from 8 physicians to 22 physicians, 9 extenders, and 480 employees. As the practice continued to grow, she was instrumental in obtaining the CON and added MRI services at 2 locations, EMG, Pharmacy Services, and In House DME Distribution with the partnership of DeRoyal. She also facilitated the acquisition of the hospital-owned shares of the physicians’ ASC such that they now own 100% in a CON state.
After the invitation to join the Ortho Forum was extended in 2011, Becky has been instrumental in Center for Sports Medicine & Orthopaedics consistently performing at the 90th percentile for key business indicators. Some of these include Revenue per FTE Physician, Operating Expenses per FTE Physician, and Ancillary Earnings per FTE Physician.
In addition to being a strong and well-respected leader in healthcare management, Becky is devoted to her family, loves to travel the world with her husband, Paul, is an avid runner, and has a special passion for cooking.

Andrew Wade
CEO, OrthoSC
Andrew Wade is the CEO of OrthoSC. Having worked in multiple clinical and administrative roles over his tenure in orthopedics, Andrew approaches leadership with a servant’s heart. He constantly seeks to advance the mission of OrthoSC — to improve and restore the lives of patients in the coastal communities of South Carolina, through vibrant, agile, and growing private practice. Outside of work, Andrew is married way out of his league, has three incredible kids, is a technology enthusiast, and an avid reader.

Ben Shah
CEO, Olympia Orthopaedic Associates
Ben Shah is the CEO of Olympia Orthopaedic Associates in Olympia, Washington. Ben started his 28 year career working in hospitals and health systems in the Portland, Oregon area serving in such roles as Director of Finance for OHSU, Director of Finance at Legacy Health and Director of Administrative Systems and Services at Legacy Health.
Ben has also volunteered as a chapter president and regional executive for the Healthcare Financial Management Association and also is the president-elect for the South Puget Sound Community College Foundation.
A proud Oregon Duck, Ben has a finance degree from University of Oregon and a MBA from Portland State University.

Richard Green
COO, Des Moines Orthopaedic Surgeons
Rich Green earned his undergraduate degree from Christian Brothers University in Memphis, Tennessee, in 1989 and his Masters of Science in Physical Therapy from Des Moines University in 1992. He was one of the original staff that founded the current physical therapy and hand department at DMOS in 1994. In 1995, he was named supervisor of the clinics, and in 2006 he assumed a similar role at MercyOne Medical Clinics. He returned to DMOS in October 2016 as Director of the PT and Hand Clinic and now serves as the Chief Operating Officer.
Throughout his career, Rich has spent countless hours volunteers in our community, most notably; as the Director of Medical Services for the Junior Olympic Games and Chief Advisor for Medical Services for the IMT Des Moines Marathon. He also taught for several years as an Adjunct Professor at Grand View University. He has presented at numerous meetings on topics ranging from general Orthopedic Physical Therapy to work site evaluations.
Rich and his wife, JoAnn, have two children – Brady and Kara, who are currently attending Iowa State University. He is an avid Notre Dame and Iowa State fan, and in his spare time, he enjoys golfing, fishing in Canada, and working his way around the three smokers he owns.

Andre Blom
CEO, Illinois Bone & Joint Institute
Andre currently serves as Chief Executive Officer for the Illinois Bone and Joint Institute LLC, a multispecialty Orthopedic practice in the greater Chicago area.
The practice consists of approximately 125 Orthopedic Physicians, 7 Rheumatologists, 6 Podiatrists, 5 Physiatrists and 225 additional Therapists and midlevel Providers with a total employee count of ~ 1,650. He reports directly to the Board of Managers of IBJI, and supervises both strategic and operational aspects of the organization.
He serves on the Value Based Committee for the OrthoForum group and has also retained his position as Director of IBJI’s “OrthoSync” program, a convener organization that serves clients in CMS’s BPCI Classic Models 2 and 3, as well as CJR models on behalf of primarily Orthopedic Practices.
Andre’s professional passion focuses on modern day healthcare redesign strategies. He believes strongly in an “agile” management style so leaders can develop and function to their fullest capacity, and teams can adapt to rapid market fluctuations in a similar fashion.
He is an advocate for transformational and transparent activities beyond the current state of healthcare, along with a renewed mindset towards truly integrated practice patterns, all designed in a Patient centric manner.
From an actuarial perspective, Andre believes that the future models for effective healthcare delivery platforms are yet to be developed. As such, one has the ability to be truly creative and innovative, as long as you operate from a sound foundational platform. Data and Informatics are key determinants of success.
Andre enjoys travelling as often as possible with his family – primarily centered around hockey trips with his daughter and rowing with his son.
Additional, he enjoys history, political sciences and cultural exploration during his travels. Generally speaking, his wife would describe him as a little bit of a sports and TED talk “nut”. He would agree but argue that it helps him focus better as part of the multi tasked world we live in.

Bruce Cohen, MD
CEO, OrthoCarolina
Dr. Cohen has served in a leadership capacity throughout his tenure with OrthoCarolina, including serving as vice-chairman of the Board of Directors and chair of the Operations Committee as OrthoCarolina established itself as a nationally-recognized model for patient success and value-based care.
An innovative thinker, he has served as Fellowship Director of the OrthoCarolina Foot and Ankle Fellowship and subspecialty chief for the Department of Orthopaedic Surgery, Carolinas Medical Center, helping lead foot and ankle innovation and education. He also has acted as a principal investigator in numerous active research projects through the OrthoCarolina Research Institute (OCRI).
He is currently serving as the Chief of the Foot and Ankle Division at Carolinas Medical Center.
Dr. Cohen, who continues to see patients through his foot and ankle practice while active as the OrthoCarolina CEO, is also a respected member of national foot and ankle research circles. He is a member of the American Orthopedic Foot and Ankle Society (AOFAS) Board of Directors, and currently serves as its Vice-President.

Doug Vang
CEO, OrthoArizona
Doug Vang is a results-oriented, seasoned Executive with a broad range of experiences throughout the healthcare industry.
Doug’s current role as CEO at OrthoArizona, a Physician owned orthopedic practice, comprised of 65 orthopedic specialists throughout the greater Phoenix metro area. OrthoArizona is recognized as the premier orthopedic practice in Arizona and is focused on excellence, quality and partnership.
Doug is responsible for all functional areas including governance, leadership, strategy, operations, finance, quality, payer contracting and marketing.
He has led unification efforts to transform OrthoArizona into an integrated group practice with all aspects of the organization. He has facilitated institutional relationships including co-management and management service arrangements with health systems. Doug has established value-based payment arrangements including bundled payment initiatives, and has developed strategic focus for enhanced market position capitalizing on our organization and physician strengths.

Karen is a certified CPA since 1991, and she earned a Bachelor of Arts in Accounting from Lynchburg College. For 13 years, Karen served as Chief Administrative Officer for OrthoVirginia-West. In 2016, Karen then served as Advocacy Chair for OrthoVirginia-State where she held responsibility for representing OrthoVirginia at the federal, state and local level regarding issues that impact the ability to deliver a vertically integrated, quality based, cost effective service to patients. Karen then served as CFO for OrthoVirginia from 2018-2019, and she currently serves as the Strategic Alliance Director for The OrthoForum.

Mike Enriquez
CEO, OSS Health
Mike has a healthcare background with experience in a large system and a private practice. He has worked for Geisinger Health System, Premier Orthopaedics in the Philadelphia suburbs, and currently serves as CEO of OSS Health where he has been for the last 9 years (5 as COO, 4 as CEO). Mike is a graduate of King’s College and has his Master’s in Healthcare Administration from Wilkes University. In his free time, Mike enjoys spending time with his wife and two daughters, Sophia and Faith.

Tom Flood
The OrthoForum
Thomas is currently a consultant with OrthoForum with current assignments related to physician and administrative staff recruitment for member groups and assisting with the Employer Direct Contracting Program development. He most recently served as Executive Director of Barrington Orthopedics, LLC, Schaumburg, IL., retiring in March of 2018. He has spent over 40 years in healthcare management in multi-specialty groups and orthopaedics. He served as the CEO for two hospital owned physician practices, Rochester, N.Y and Elk Grove, IL. Mr. Flood and his wife live on Tellico Lake in Loudon, TN. He enjoys family time, boating, golfing and his new video editing volunteer work.

Barbie Hayes
CAO East/West, OrthoVirginia
Barbie has worked in Orthopaedics for the duration of her professional career which began in 1997. She participated in two mergers, the most recent being with OrthoVirginia in 2016. She has spent most of her career in operations, which allowed her to the opportunity to successfully lead the implementation of the Epic system across 23 offices and 1500 employees in just under a year and half. Barbie is currently the Chief Administrative Officer for the Eastern and Western regions of OrthoVirginia and is currently working to expand OrthoVirginia into other areas of Virginia including south west Virginia and the Hampton Roads area. She graduated from the University of Virginia with a BA in Psychology and with an MBA from Averett University.

Don Schreiner
CEO, OrthoIllinois
Don has 21 years at OrthoIllinois starting at COO to current CEO for the last 16 years. He holds a Bachelor’s in Business and an MBA from University of Toledo. Don is past Board member 2008-2016 and Board President of AAOE from 2014-2015. Don is a current Board member of The OrthoForum.

Anthony Shaia
MD, CMO, OrthoVirginia
Tony is a practicing orthopedic surgeon with OrthoVirginia. His practice is focused on total hip and knee replacement surgery. He is am CMO and direct insurance contracting for the group.

Mike Humphrey
CEO, Syracuse Orthopedic Specialists
A native and resident of Upstate , NY for my whole life. Graduate of Utica College with degree in Accounting. CPA with 15 years of Hospital financial and management experience prior to joining SOS. Have been the CEO of SOS for 17 years.

Kevin Joyce
CEO, Orlando Orthopaedic Center
Kevin Joyce is the Executive Director of Orlando Orthopaedic Center which currently has 21 physicians, 8 mid-level providers, 6, soon to be 8, Central FL locations as well as a network of physical therapy and MRI facilities. Orlando Orthopaedic Center employs approximately 250 employees between their clinics and their state of the art orthopaedic surgery center located in downtown Orlando.
Mr. Joyce joined Orlando Orthopaedic Center as Executive Director in 2006. Accomplished in orthopaedic practice management, Mr. Joyce’s experience includes serving as the Executive Director of Orthopaedic Associates of St. Augustine and Administrative Director of the University of Florida Department of Orthopaedics at Shands Jacksonville.
On the forefront of medical practice management, Mr. Joyce has successfully passed his board certification with the American College of Medical Practice Executives and is active in related societies including Medical Group Management Association and the American Association of Orthopaedic Executives. Kevin has extensive experience with the implementation of electronic health record systems and Digital X-ray. He has also directed the construction and development of orthopaedic ambulatory surgery centers and pain management programs. He has also helped develop many successful and profitable ancillary services within the orthopaedic practice including in-house brace dispensing, medication dispensing, interventional pain management services, MRI and Physical Therapy.
Prior to orthopaedic practice administration, Mr. Joyce implemented the start-up of a new healthcare Preferred Provider Network (PPO) and served as Director of Provider Networks Premera Health Plans and KPS Health Plans in Washington State.
Mr. Joyce attained his Bachelor’s degree from Ithaca College and his MBA with emphasis in Health Care Administration from Temple University. He extended his graduate work through administrative fellowships at The John Hopkins University and also with the National Association of Community Health Centers.

Sani Mirza
CEO, TSAOG Orthopaedics
Usman (Sani) Mirza has been Chief Executive Officer of The San Antonio Orthopaedic Group since 2000. He has been in healthcare since 1987 and has worked in a hospital system, served as a leader in the strategic planning service for one of the largest healthcare consulting firms and in an academic medical center. Over the past 20 years while being the CEO of TSAOG Orthopaedics, he has also simultaneously served as CEO for two other large orthopedic groups. While serving as the CEO of TSAOG Orthopaedics, the organization has grown from 9 orthopaedists to 45, with seven full-service clinics and two eight-operating room ASCs. He has also negotiated several long-term multi-hospital orthopaedic service line agreements with one of the largest Health Systems in the San Antonio region and has worked with them in creating a state-of-the-art orthopaedic specialty hospital that has 10 operating rooms and 30 beds, which opened in November 2016.
